Medical Errors: The Silent Killer in Medicine | Carol Gunn | TEDxFargo
Dr. Carol E. Gunn shares a powerful story about her sister and the obstacles they faced due to errors in healthcare. She openly wonders why, when medical errors are the third leading cause of death, there is little outage, transparency or accountability. In her unique position of being both a physician and a family member who has suffered a great loss, she understands the tremendous pressures facing both providers and chronically ill patients. Medicine is Dr. Gunn’s second career. Prior to medical school, she worked as an environmental health and safety professional … which taught her the skills of accident investigations and root cause analysis. She began medical school later than most, at 37 years of age. She then completed medical school, a primary care residency, and her occupational and environmental medicine fellowship. Dr Carol Gunn, an internist and occupational medicine physician, thrives on working with individuals and groups advocating for patient and employee safety. She has been an agent of agent nearly her whole life. After completing her bachelor’s in chemistry and master’s of public health degrees, she was a safety engineer, during her first career. For twelve years, she focused on reducing workplace exposures to harmful chemicals. With the goal of creating greater impact on others’ health, she then elected to attend medical school. She completed an internal medicine residency and a fellowship in occupational medicine, opening her own independent practice in 2007. She is boarded in internal medicine, occupational and environmental medicine, and industrial hygiene. Outside of medicine, she lives with her wild Lab, and enjoys the varied activities of the Pacific Northwest. She valiantly attempts to balance her life of family, work, and play - but there never is enough play! Your future is nothing but the outcome of the decisions you make today. You make better decisions today, your future will be more prosperous, you make the wrong ones, then your future will be full of struggles. So, today we are going to talk about critical thinking and how it applies to problem-solving. I’ll give you a weird analogy here. Critical thinking ability is like having infrared goggles and looking at the sky at night. Without it, You look at the space, you see only three things, you see darkness, stars, and sometimes planets. But the moment You put on your infrared goggles then you get to see all these beautiful things that were invisible before. You see giant gas clouds in the galaxy, you see dust, you see all these things that were previously impossible to see. That’s what critical thinking is. It lets you identify the actual problems, the root causes, but also it helps you see the opportunities. Now, critical thinking isn’t only applicable to your work. It’s applicable to every single area of your life. But my channel is all about your career. So, that’s what’ll cover in this video. Now, we need a framework. And the framework starts with Problem Statement. The problem statement is very similar to a project charter. It includes; Goals, as in what are you trying to achieve, your success criteria, as in how will I know I succeeded or failed, your assumptions, timelines, and stakeholders involved. It’s very similar to a project charter. I am actually going to call this Problem Charter. This document is very helpful for two reasons. The second reason being a lot more important than the first. The first reason why it’s very helpful is that when you get engaged in that problem solving mode, you start uncovering a lot of other symptoms, that may be caused by completely other root causes. We are not interested in that at that moment. You will document those, but you are not going to develop solution alternatives and develop action plans for those. Unless there are dependencies. So, it helps you understand your scope, who to deal with, the timelines, it basically keeps everything under control. But to be honest, in my decade long consulting career, I noticed that the biggest help of having such a charter is all about moving through the bureaucracy. Let me explain. When you are going through problem-solving stages, you actually do a lot of work, you summon meetings, you request data from various departments, you request for expertise from consultants, so you are shaking things, you are moving things around. Now, what gives you the power to do it? What gives you the power to call all these people to a meeting room, what gives you the power to ask for certain analytics data from a completely different department? Why should they attend that meeting or give you the data you want? Because they like you and they want to help you? What if they don’t like you. Now if you are a senior employee, you are a manager, director, VP, then fine, of course, everyone will come into the meeting or give you the data you want. But if you are not that senior. Where does your power come from? It comes from that document. The problem charter. Corporate companies aren’t usually very agile. They move slowly and the employees are usually very – they are not lazy but they push back –They don’t want to stop what they are doing and join your little problem-solving brainstorm session and go through your little power point of fishbone analysis. They got their own thing to worry about. So, if you have your problem charter, only a few pages, signed off by a sponsor, someone senior, then you have the power to get the data you want and bring in experts and call for meetings. Ok, now let’s continue; I mean you developed the problem statement, you got your buy-in from someone senior, now, you are off to solve the problem. The 2nd step in your critical thinking towards a problem is identifying the root causes, right? 5 Whys is made famous by Toyota Production System. As the name suggests, you ask WHY WHY WHY until you get to a root cause. Very simple stuff. An error was found at the end of the nurse's shift when the narcotics count showed an extra Clonazepam and a missing Lorazepam. No harm was done to the patient. Find sketches here: http://www.bookvideoclub.com/blog (Sign up to our email list) --Introduction-- In “The Manager’s Communication Toolkit: tools and techniques for leading difficult personalities”, Tina Kuhn identifies 10 difficult personalities in the workplace, and shows through examples the wrong AND the right way to deal with them. --The big idea-- To accomplish pretty much anything, you need the work and help of others. So you HAVE to motivate and inspire. Top performers are consistently those who can get the most out of their teams. And to do that, you’ve GOT to communicate effectively. But how can you deal with people who are liars, gossipers, or manipulators? When dealing with difficult personalities, your first reaction may be to confront them and demand that they stop their harmful behavior. But that’s NOT going to work. Instead, focus on identifying and then attacking the root cause of the problem… and the bad behavior will go away. --How to deal with difficult personalities-- Take a gossiper for example. Gossip is rampant in most workplaces, and it can be detrimental to morale, cause miscommunications, and create a toxic culture. Sure you can pull the gossiper aside and demand that they stop immediately – you’re onto them! You may think you’ve dealt with the problem but it will probably fuel much more gossip… You can do much better by looking at WHY they're resorting to gossip. Maybe they’re protecting themselves from something, or maybe they want to feel more important. --How to eliminate the root cause of bad behavior-- So how can you eliminate the root cause of the problem? With a gossiper, it’s a good idea to take more of a coaching approach. Make it clear that their behavior is harmful, and coach them out of that habit, one-on-one. Or take the naysayer, who questions every decision and always comes up with reasons why things CAN’T be done. They probably want to feel like they’re being heard. So when you deal with them, keep the discussion on the facts and stay away from opinions. Then, give them face-saving opportunities to change their minds, without feeling they’re being judged. And what about the drama-queen - the person who thrives on attention and spins small issues into disasters? Ask them to summarize what happened in one sentence. That will force them to get down to the facts. --Conclusion-- To become a great leader, you have to become an effective communicator first. Between your team and your vision stand many challenging personalities that can derail communications. But if you learn how to deal with them, listen to them, and understand what’s driving the bad behaviors, you can weed them out, empower your people, and align your team. --------------------------------------------------------------------------------------------- Like what you see? The Root Cause Documentary REMOVED From Netlix

Quietly is the word that we use to see it go from Netflix, but I'm sure the American Dental Association was not quiet in their condemnation in legal threats against Netflix and other platforms that wer showing the documentary. The American Dental Association, Ada, American Association of endodontist and American Dental research Association said take it down so they took it down immediately. If there was really something wrong with this documentary they should have refuted the facts. They should have ridiculed it. They should have shown where the documentary was wrong. Instead they just demanded it be taken down. Pretty fishy. The Ada is the same organization that gives us fluoride in our drinking water whether we want it or not and then tells us it's great for us. This is the same organization that tells us that mercury fillings are okay to put into are teeth in our mouths and yet it is listed as a hazard by the FDA so the Ada no longer endorse is it. The health information Nazis have reared their ugly heads and they won this time. From the Guardian: Netflix has quietly pulled the controversial documentary Root Cause from its streaming platform and scrubbed all trace of it from their website.   "Netflix pulls controversial documentary that claims root canals cause cancer" - The Guardian Netflix began carrying the film 1 January 2019, and it soon met opposition from the American Dental Association (ADA), American Association of Endodontists (AAE) and American Association of Dental Research (AADR). In a letter dated 29 January, the ADA, AAE and AADR wrote that continuing to host the film on their platform could harm the viewing public by spreading misinformation about safe medical treatments like root canals. hese associations also sent similar letters to Apple, Amazon and Vimeo, which also carried the film. When testing high value or mission critical devices it is often desirable to track or record all power related events during testing. There are two common reasons for keeping a power record during test: 1) If the device under test (DUT) is damaged during test the power record can be used for forensic purposes to determine what caused the damage to ensure it does not happen again 2) To ensure the DUT did not experience a condition, such as a high current spike, that could put its reliability in question. The record allows you to identify such a condition and allows you to avoid using the DUT in a mission critical application. Capturing or continually logging such a power record during test is not an easy task. Depending on what needs to be tracked, keeping such a record can add significant development time and complexity to a test system's hardware and software.
