I demonstrate how the GMP 050 to 100 model furnace heat exchanger failure diagnosis is done. These furnaces were installed in the early 90s and virtually all of their heat exchangers failed. Most were replaced within the 15 year warranty period. However, some of them were replaced twice. I recommend if you come on to one of these to replace the furnace. There is a possibility of CO poisoning from this appliance. Pneumothorax nursing interventions, treatment, pathophysiology NCLEX review for the different types of pneumothorax: open pneumothorax vs closed pneumothorax vs tension pneumothorax. A pneumothorax is the collection of air inside the intrapleural space which causes the lung to collapse. There are various types of pneumothorax. An open pneumothorax (also called sucking chest wound) is an opening in the chest wall (from a gun shot, stabbing etc.) that causes a passage of outside air betwen the intrapleural space which allows air to pass back and forth during inspiration and expiration. A closed pneumothorax is when air leaks into the intrapleural space without any outside wound, hence the chest wall and pleural stay intact (could be from a rib fracture where the bony part of the bone punctures the lung which causes air to be released into the intrapleural space). A tension pneumothorax is a complication of a pneumothorax due to an open or closed pneumothorax. It is a medical emergency. It happens when the opening to the intrapleural space creates a one-way valve which causes air to collect in the intrapleural space and the air cannot escape. This causes major compression on the lungs and heart leading to a mediastinum shift and tracheal deviation. Pneumothorax nursing interventions include maintaining chest tube drainage system, monitoring lungs sounds, vital signs etc. Don't forget to watch the video on chest tube care for nurses. As I said in the video I've been asked about this video for some time and I was happy to get the opportunity to make it. Now that I think about it however I did not address ABS systems in this video however the Subaru used does have ABS. For the most part, you can replace the brake fluid and not have to worry about the ABS, however in some cases you might need to bleed the system with a scan tool in order to get the brakes to work properly. Be sure to check the service manual of the vehicle you're working on before committing to bleeding at the wheels. If you're just removing the fluid in the master cylinder, you should not have to worry about the ABS at all. It consists of specialized muscle cells called cardiac myocytes. The contraction of these cells is initiated by electrical impulses, known as action potentials. The impulses start from a small group of myocytes called the PACEMAKER cells, which constitute the cardiac conduction system. The cells of the SA node fire SPONTANEOUSLY, generating action potentials that spread though the contractile myocytes of the atria. The myocytes are connected by gap junctions. This enables electrical coupling of neighboring cells. Pacemaker cells and contractile myocytes exhibit different forms of action potentials. The pacemaker cells of the SA node SPONTANEOUSLY fire about 80 action potentials per minute, each of which sets off a heartbeat. Pacemaker cells do NOT have a TRUE RESTING potential. The voltage starts at about -60mV and SPONTANEOUSLY moves upward until it reaches the threshold of -40mV. This is due to action of so-called “FUNNY” currents present ONLY in pacemaker cells. Funny channels open when membrane voltage becomes lower than -40mV and allow slow influx of sodium. The resulting DE-polarization is known as “pacemaker potential”. Calcium channels open, calcium ions flow into the cell further DE-polarizing the membrane. This results in the rising phase. At peak, potassium channels open, calcium channels inactivate, potassium ions leave the cell and the voltage returns to -60mV. This is falling phase of the action potential. Contractile myocytes have a different set of ion channels. Their sarcoplasmic reticulum, the SR, stores a large amount of calcium. They also contain myofibrils. The contractile cells have a stable resting potential of -90mV and depolarize ONLY when stimulated. When a cell is DE-polarized, positive ions leak through the gap junctions to the adjacent cell and bring the membrane voltage of this cell up to the threshold of -70mV. FAST sodium channels open creating a rapid sodium influx and a sharp rise in voltage. This is the depolarizing phase. L-type, or SLOW, calcium channels also open at -40mV, causing a slow but steady influx. Sodium channels close quickly, voltage-gated potassium channels open and these result in a small decrease in membrane potential, known as EARLY RE-polarization phase. The calcium channels remain open and the potassium efflux is eventually balanced by the calcium influx. This keeps the membrane potential relatively stable for about 200 msec resulting in the PLATEAU phase, characteristic of cardiac action potentials. Calcium is crucial in coupling electrical excitation to physical muscle contraction. The influx of calcium from the extracellular fluid triggers a MUCH greater calcium release from the SR, in a process known as “calcium-induced calcium release". Calcium sets off muscle contraction by “sliding filament mechanism”. Calcium channels close, potassium efflux predominates and membrane voltage returns to its resting value. The absolute refractory period is much longer in cardiac muscle. This is essential in preventing summation and tetanus. It is the fourth leading cause of death in the United States according to CDC. Pathophysiology To better understand the disease process of COPD, let’s go back to the normal anatomy and physiology of the respiratory system. Every time we inhale, air goes in and passes through the windpipe which divides into 2 branches known as bronchial tubes, and each one of these enters one of the two lungs in the body. Each branches resembles the limb of a tree dividing into thousand smaller, finer branches called bronchioles. The bronchioles ends in a tiny air sacs called alveoli, they are like tiny balloons that stretch and fill with air when you breathe in and shrink back when you breathe out. These air sacs are surrounded by tiny blood vessels or capillaries where the process of gas exchange takes place. Once the air we breathe in reaches the air sacs, oxygen passes through the air sac walls into the blood in the capillaries. At the same time, carbon dioxide (waste gas) moves out from the capillaries into the air sacs to be breath out. So what happens with COPD? In COPD, less air flow in and out of the airway because of one or a combination of the following reasons: The air sacs and airways their stretching ability The walls of the air sac are destroyed The walls of the airways became thick or inflamed The airways becomes clogged with mucus There are two main forms of COPD, that causes the following reasons mentioned earlier. Chronic bronchitis and Emphysema. In Chronic bronchitis, the lining of the airways (bronchi) is constantly irritated and became inflamed and swollen, making the airway narrow and clogged with mucus, called phlegm, making it hard to breathe. With emphysema, the walls between many of the air sacs are destroyed. As a result, the air sacs lose their shape and become floppy, the damage can also lead to fewer and larger air sacs instead of many tiny ones. When this happens, the amount of gas exchange in the lungs is reduced. In most cases, chronic bronchitis and emphysema can be found together. Thus, the general term “COPD” is more accurate. Causes Long term exposure to lung irritants that irritates and damage the lungs and the airways overtime is the cause of COPD. Smoking as the main cause, while exposure to air pollution, or chemical fumes or dust from the environment or workplace are the less common cause that contributes to the development of COPD. In rare cases, a genetic condition called as Alpha-1 antitrypsin deficiency may assume a part in causing COPD. Individuals with this condition has low level of alpha-1 antitrypsin (AAT) –a protein made in the liver. This condition can lead to lung damage

Educational video describing classification of open fractures of the tibia. Open fractures occur when the fractured bone penetrates through the skin. Open fractures typically occur by high energy injuries such as car crashes, falls, or sports related injuries. The Gustillo-Anderson classificaton of open tibial fractures describes the amount of enegry, the extent of soft-tissue injury and the extent of contamination or determination of the open fracture severity. There are three types of open fractures: •Type I: low energy clean wound that is usually less than 1 cm in length with minimal soft tissue injury and minimal bone contamination. •Type II: wound length 2-10 cm with moderate soft tissue damage and wound contamination. •Type III: high energy wound usually greater than 10 cm in length with extensive muscle devitalziation. Wound is highly contaminated with extensive soft tissue damage. Type III fractures can also include open segmental fractures, open fractures in a highly contaminated environment such as in farm yards or associated with high velocity gun shot wounds. These wounds are classified as type III regardless of the size and level of contamination. Adequate emergency debridement of the devitalized tissue is required to prevent wound infection. Type III is further classified into: Type III A: severe soft tissues injury, soft tissue coverage of the bone is possible. Type III B: severe loss of soft tissue loss, bone coverage is not possibile. Maybe moderate to severe bone comminution. Usually requires a local or distant flap. Open fractures in the middle part of the tibia can be reconstructed by using a soleus muscle flap. Open fractures in the proximal part of the tibia can be reconstructed by using a medial gastrocnemius muscle flap. Open fractures in the distal part of the tibia can be reconstructed by using a latissimus dorsi free flap. Type III C: vascular injury requiring repair or amputation. Example of type III C proximal tibial open fracture with injury to the popliteal artery. Amputation is required if: •Extensive vascular injury •Muscle & tissue ischemia of more than 8 hours (Warm ischemia time) •No plantar sensation due to posterior tibial nerve injury •Can use mangled extremity severity score. Steps for How to Diagnose Leaks in a Steam Boiler: 1. Set the thermostat at a high temperature to turn on the boiler, and leave it running during diagnosis. 2. Once the boiler has turned on, check the floor around the boiler. This would indicate a leak on the water side of the boiler. 3. Check all exposed piping, especially at the joints for a leak. 4. Check the main vent of the system to locate a leak. 5.If there are no leaks near the boiler, check the radiators in each room. Look for signs of water around the hand valve, at the connection of the steam pipe to the radiator, and at the air vent on each radiator. 6. If there are no obvious places where water is leaking, shut off the boiler and allow it to cool. When the boiler has cooled down, remove the vent pipe and inspect inside for any corrosion on the boiler itself. 7. Work with a qualified plumber or HVAC technician to repair any damaged or leaking areas. DIY (Diagnose It Yourself) Medicine Begins with an Open Source Reinvention of the Stethoscope

The stethoscope... one of the most iconic tools used by physicians... a barrier patient-centered care. There is definitely nothing patient-centered about a clinician putting a stethoscope in his or her ears and proceeding to command a patient through a series of position changes and deep breaths only to conclude with "everything sounds fine" or "hmmm". For patients to become truly active participants in their care, we not only need to rethink our communication and educational tools but also every medication instrument that we use. Sure, there are some patient-friendly versions of pedometers, sphygmomanometers, and glucometers coming to market, but what about the time-honored clinician tools like the otoscope, ophthalmoscope, and stethoscope. The patient should be able to hear everything that the clinician hears and see everything that the clinician sees. The instruments should be in every patient home as well. They should be inexpensive and easy to use. Even more important, they need to be reinvented so that they will fit into a modern model of healthcare delivery. They need to be networked with the ability to remotely control them and for them to stream data securely over the web. Patient and coach should be able to explore the data together and make comparisons not only over time but also across populations. Yadid Ayzenberg of the New Media Medicine group has started this endeavor with the stethoscope. He is building a new tool that will not only meet the goals described but that will also be completely open source. Everything about the device from the shell to the electronics to the software will be fully documented and shared openly with the world. Each of the components is being specifically chosen so that the entire device can be fabricated at an extremely low cost with tools that are already available in fab-labs around the world and that you may soon have in your garage. Your next DIY project might be to build your own stethoscope (of course one that only faintly resembles the one that your pediatrician used to use... you know, the one with a fuzzy little animal glued to it so that it would appear more "patient-friendly").
