7-Bolt Shortblock Failure - Full Diagnosis

If you are your own mechanic, there is no more important character trait worthy of development than the ability to own your mistakes. That's where the line is drawn between good mechanics and bad mechanics. It's not the failures but how they deal with them that measures their ability. In short, it's not easy to admit you did something wrong or were negligent. But if you don't own it and talk about it, it doesn't get fixed, and nothing positive can come from it. It was my quest to overcome my clutch issue that lead to the creation of a video. That video is the textbook perfect guide for how to correctly install a DSM transmission. https://www.youtube.com/watch?v=6bE_9sWtnSY&list=PL4B97C16D423317DD Crankwalk as described is caused by a casting defect. This was not a defect. This was preventable. A lot of people would find something like this and not tell anyone out of embarrassment. I'm not ashamed. It's my fault. I got good use out of this engine and it was tough enough to make it 48K miles since the last rebuild despite my abuse. I'm here to tell you if you bought a used car that's had its clutch replaced, or if you ever pay someone else to do it... make sure it has this bolt. It's stashed away between the starter and the transfer case, so it's hard to see. Make sure all of your bell housing bolts are torqued properly because fastener problems can destroy your shortblock, clutch and transmission. If your car fails because of a mis-aligned transmission, you have no reason to blame crankwalk. It wasn't until I bought my next AWD car that I discovered there was a smaller bolt on the other side of the block. I destroyed 3 transmissions in the GSX first. With the damage already done to my crankshaft, I then lost a shortblock. It's an ounce of prevention that's worth metric tons on your bank account. Grade 10 M8x60 bell housing bolt = MD706012. It gets 22-25'lbs of torque. Owning my mistake permits me to learn from it through con$equence$, and never repeat it. What good would it have done anyone else for me to learn this lesson and not share it? That's why I'm providing this video to all of you. Sharing it can perhaps help someone else avoid this costly mistake. This is the final chapter for my 7-bolt, and this book is going back on the shelf. Whether the casting defect exists?... or it's all caused by a bolt, or the harmonics, or whatever... Sure, crankwalk exists and it's horrible. But with the small amount of movement required for your crankshaft before it contacts the block isn't far enough to make your clutch drop to the floor when you turn. You'd be hearing woodpeckers and jackhammers on the crank long before that clutch pedal would fall to the floor. Some people are going to hate on me for saying that. That's fine. I believe all of the people who experienced the clutch pedal issues had fastener problems on their bell housing. DSMs get a bad reputation for this but we can change that. Crankwalk is never the cause of your engine failure. Crankwalk is always a symptom of the real problem. It's your disease that makes you deny it's your fault. You've got the 'itis. DSM-itis. Whenever you dig deeper, you'll discover what applied all of those thrust loads to your crankshaft to begin with, and it's not going to be a casting defect that moves your crank .101". Mine only went .014", but all of the same parts failed. PLEASE tell me in the comments if you find this bolt is missing from your car.

Once again I dove into the comments section to see what medical tv drama you wanted me to review next and it seemed like Dr. House or House M.D. was the next most popular choice. Get ready TV Doctors, because Dr. Mike is coming for you =] Full disclosure, I have watched a few episodes of House before but this was probably 10 years ago so I decided to go ahead and start this tv medical drama review off with the first episode ( House MD episode 1 ) of the series. I was really curious to see the difference in watching the show now that I am a licensed doctor. The last time I watched the program I was in high school and knew a tenth of the medical knowledge I know now! Honestly, I love this medical tv show series because I think it allows us to laugh at some of the crazy situations Hugh Laurie gets himself into. Plus It gave an interesting look at what life is like as an infectious disease doctor especially one as highly regarded as Dr. House. Obviously, the way that he practices medicine is not realistic or accurate because we don't break into people's homes nor do we treat them without explaining what we are doing. All in all, I think Dr. House is a must watch tv show if you're into medical dramas just for laughs alone. I was really curious myself to see if the good doctor is a medically accurate show. I really did enjoy this medical tv show because I think it did a lot of things well. It gave an interesting look into what life is like a for a surgeon and a resident. Being a surgical resident is not an easy job and to make the situation even more complex Dr. Sean Murphy has autism so they did a great job of breaking down these circumstances. In the UK we have a very good NHS Site Online which can be a good starting point for self-diagnosis but the Internet has many sites such as health Forums and Fake Doctors which may cause panic when self-diagnosis is looked at. That is not to say there are genuine people trying to help through past experiences. Back in the day, of course, many people used The Family Medical dictionary and that was equally scary with self-diagnosis usually resulting in two weeks to live. A recent TV programme (Jeremy Vine ) discussed the issue and I thought there's got to be a song in that topic. In the final analysis, seek professional help and keep off the net when it comes to ill health. You'll only make your anxiety worse.

Final Analysis of the Phase 1a/b Study of Chimeric Fibril-Reactive Monoclonal Antibody 11-1F4 in Patients with RR Amyloidosis Suzanne Lentzsch, MD, PhD Columbia University Medical Center New York, NY, USA Background: AL amyloidosis has a very poor prognosis with a variable median survival of 12 to 18 months depending largely on the severity of organ involvement. Current treatment focusing on eliminating the plasma cell clone that produces amyloidogenic light chains have improved overall survival in these patients [Kumar 2011]. However, the risk of early mortality remains high due to multi-organ dysfunction caused by persistent, insoluble amyloid fibril deposits. To address this, amyloid fibril-reactive monoclonal antibody (mAb) 11-1F4 was designed to target amyloid deposits by directly binding to a conformational epitope present on human light-chain amyloid fibrils. The mAb was tested in a murine (Mu) and later a chimeric (Ch) form in mice with induced human AL amyloidomas; to which there was rapid destruction of amyloid fibrils without any evidence of toxicity in the animals [Hrncic 2000; Solomon 2003]. Confirmation of the mAb’s specificity for amyloid fibrils was further demonstrated when the I-124 labeled Mu mAb was visualized in amyloid-laden organs on PET/CT imaging in human subjects [Wall 2010]. These promising results led to the development of GMP-grade amyloid fibril-reactive chimeric IgG1 mAb 11-1F4 by NCI’s Biological Resource Branch. Here we report the final data from the open-label, dose-escalation phase 1a/b study of Ch IgG1 mAb 11-1F4 (NCT02245867). Conclusions: We found that mAb 11-1F4 is well tolerated and safe without grade 4 or 5 AEs nor dose limiting toxicity (MTD of 500mg/m2). Moreover, we postulate that in patients with persistent organ dysfunction after plasma-cell directed therapy; mAb 11-1F4 leads to fast, early and sustained organ response. Overall, amyloid fibril targeted therapy with mAb 11-1F4 represents both a promising and innovative approach to the management of patients with AL Amyloidosis. The rapid destruction of amyloid fibrils by mAb 11-1F4 can improve organ function and, potentially improve mortality in patients with this uniformly fatal disease. Larger randomized trials to evaluate the efficacy of this mAb are planned. A millimeter-sized, low-cost medical diagnostic chip could help provide remote healthcare locations with technologies for diagnosing infectious diseases and health conditions. The new technology developed by Kaushik Sengupta and his team at Princeton University detects pathogens and disease biomarkers by integrating optical structures and electronic systems into sophisticated, fluorescence-based sensors and scanners. Manufactured using the same process for making the chips that power today’s smartphones and other electronics, this technology can drastically reduce the cost of medical diagnostics, a rapidly growing area in health care. Molecular diagnostic devices detect and measure nucleic acids and proteins, which can be biomarkers for specific health conditions across a range of bacterial, viral and parasitic infections. However, today’s diagnostic methods involve collecting blood or other biological samples and shipping them to a laboratory for analysis by skilled technicians using fluorescence-based optical scanners, which are bulky, expensive and nonportable. The device invented by Sengupta and his team enables the analysis of multiple molecular signatures simultaneously, making portable and low-cost diagnostic technology available at the point of care for use by personnel who have had relatively little training. Such a device could facilitate rapid testing for emerging diseases and outbreaks, thus shortening the response time of medical and public health officials. The diagnostic system is made possible by work in the Sengupta lab that incorporates novel optical nanostructures in silicon-based integrated circuit technology to manipulate, control, filter, detect and process signals of light in the visible range. This system eliminates bulky external optical components, and miniaturizes the entire multiplexed fluorescence scanner, including the optics and electronics, in a single chip. When the biomolecules are tagged with optical labels, the computer chip can detect and quantify them with high precision, thus identifying pathogens and other disease-causing agents. With the ability to be powered by a lithium battery, this miniaturized diagnostic device makes portable biosensors a real possibility for the future of medicine and personalized health care. Team members include Lingyu Hong, graduate student in the Department of Electrical Engineering, and collaborators Haw Yang, professor of chemistry, and Hao Li, a graduate student in the Department of Chemistry.
